Services available for HACC at the EGCC include:

All HACC services delivered by the EGCC are carried out by supporting the “Wellness Philosophy”, which recognises an individual’s abilities, promotes independence, supports current skills, assists in regaining lost skills and supports community connections thereby encouraging independent living in the community.
Meals on Wheels as part of the HACC service is also based at the centre, where meals are prepared in accordance with strict food safety guidelines, chilled, packed then delivered by a team of volunteers.
Meals for Meals on Wheels are prepared and delivered from Monday to Friday and consist of soup and a main meal.
RAS will explore your eligibility for the WA HACC Program. RAS works in partnership with the Aged Care Assessment Team assessors and Allied Health Professionals to ensure people access the most appropriate level of support.
A RAS assessor will conduct the assessment, commence support planning and facilitate access to support within the WA HACC Program and the broader community care sector. Assessors work with the individual and, where appropriate, their carer to explore their goals, abilities, needs and support strategies.
A referral to RAS can be made by a family member, carer or health care professional. The Eastern Goldfields Community Centre can assist with this or the person can contact RAS themselves.
